脊髓损伤晚期病例病理解剖特点，主要是由于椎管前方的骨性致压物妨碍其神经功能的进一步恢复。因此，对于颈椎骨折脱位合并脊髓伤者，以颈前路扩大性减压术最佳；而胸腰椎骨折脱位合并不全瘫者，则以次全环状减压术更为理想，因其减压范围充分、损伤小及对脊柱的稳定性影响较少，其不仅可以同时对椎管的侧方和前方减压，如果需要的话，尚可行蛛网膜下腔松解术。彻底减压有助于脊髓及其血管功能的恢复。
